Library ADA Exam Accommodation Space Request Form

To request library space for ADA testing accommodations, please complete this form. Library staff will review available spaces and confirm reservations for faculty members or their designated testing proctors.

Please note that library testing spaces are reserved exclusively for students with approved ADA testing accommodations and are not available for routine testing, makeup exams, quiz administration, or other non-ADA testing situations. This helps ensure that limited testing spaces remain available for students who require approved accommodations.

Faculty members or their designees are responsible for communicating all examination instructions, materials, timing requirements, and testing parameters directly to students. To support exam security, faculty are encouraged to utilize ExamSoft, Respondus, or other approved testing security measures when appropriate.

While library staff will assist with room reservations, they do not proctor, monitor, or administer examinations. Responsibility for exam oversight remains with the faculty member or their designated testing proctor.

Students may not schedule ADA testing accommodations independently. All requests must be submitted by the faculty member or their designated testing proctor.

Basic Information

Faculty should follow program-specific guidance for the timing and facilitation of structured examination breaks, including restroom use and testing security procedures. A faculty member or program designee should be present to facilitate these breaks and respond to student questions. Unless a student has an approved accommodation or is in a designated structured break, students should remain in the testing environment for the duration of the exam and may leave only in the event of an emergency.

Faculty or a program designee are also responsible for coordinating any additional testing accommodations or needs that fall outside of Library-provided space, including setting up and managing the appropriate proctoring platform or technology (e.g., ExamSoft/Examplify, Respondus).
